I'm not sure about the website design part, but I was quite happy learning
PHP from "Programming PHP", and MySQL from "MySQL Cookbook" (both published
by O'Reilly), as well as "MySQL Tutorial" (published by MySQL press).

After that, the online docs for both have gotten me through most of my
issues.  php.net is especially easy to search.
