If all you are wanting to do is serve images off a disk quickly, look  
at using something like boa.

If you're pulling them out of a db, you might consider memcached. But  
I've never been a big fan of storing such in db's.

And if I'm misunderstanding what you're trying to do or why you were  
looking at mod_cache then a little more detail would be useful.
