I use a camera I bought from Walmart made by Philips which is a desk mounted USB camera (about $25). I then took a lens from a door peep hole from Lowes and glued it directly to the camera lens and it focuses at about 1-2 inches. Not too much depth of field, though. Does have a light for close up work.

Another way to make a camera work is to get one of the expensive ones for photography. The lens will focus down to a couple inches away. As far as how to make it give you a live picture, I don't know. The light also is a problem. Matt Linehan (works with us) has a close up LED light for his camera, which puts out plenty of white light for pretty darn good close ups.
